Upperline Health is seeking a Care Navigator to support a team of clinicians in coordinating care for Upperline Plus patients. This remote role involves telephonic coordination, scheduling visits, managing referrals, and assisting with preventative care tasks.

Ideal candidates have associative degree or higher, healthcare coordination experience, and strong customer service and organizational skills. Remote work is available for multiple states, with standard Monday–Friday hours CST.”

Formación

• Associates degree or higher education is required.
• BSW, CNA, LPN, or MA certification is highly preferred.
• Healthcare experience coordinating care for chronically ill patients is required.
• Bilingual English/Spanish is preferred.

Responsabilidades

• Conduct telephonic communication with patients and families, triaging calls and proactive outreach.
• Develop and manage patient care plans with patients to meet health goals.
• Coordinate care between patients, families, and providers for clear communication.
