Eon Patient Management (EPM) seeks a Navigator I in Denver, a non-clinical, administrative role central to moving patients through a complex healthcare system. You will coordinate care, communicate with patients and offices, and document interactions in the EPM platform.

Qualifications

• Associate degree or higher in a healthcare-related field is preferred.
• Healthcare experience required (minimum 2 years in a medical setting preferred).
• Foundational knowledge of medical terminology (required).
• Proficiency with Apple MacBook and Google Suite (preferred).
• Excellent verbal and written communication skills.
• Ability to work in a fast-paced, high-volume environment.
• Highly organized with the ability to manage multiple tasks and meet deadlines.

Responsibilities

• Conduct structured outbound calls to patients and providers to coordinate overdue or upcoming follow-up care.
• Handle inbound calls from patients and provider offices, using approved protocols to respond with empathy and professionalism.
• Help patients overcome logistical barriers to care (scheduling confusion, understanding next steps, connecting with offices).
• Follow standardized call scripts and workflows to ensure quality, compliance, and patient/provider satisfaction.
• Accurately document all interactions, call outcomes, and update patient records within the EPM platform.
• Identify and elevate discrepancies, unclear findings, or complex inquiries to the appropriate healthcare provider or supervisor.
• Collaborate with internal teams to uphold data accuracy, system integrity, and efficiency.
• Support internal process improvement and participate in quality assurance initiatives.
