Job summary

UnitedHealth Group is hiring a Care Coordinator to welcome patients, coordinate health enhancement programs, and ensure continuity of care across the continuum. The role supports a patient-centered Health Enhancement/Education team and emphasizes data gathering, referrals, and effective communication.

The position offers remote work from anywhere in the U.S. with some locations requiring in-office presence four days weekly.

Qualifications

• High school diploma, GED, or equivalent.
• 1+ years and including 3+ years of experience in a healthcare setting.
• Health literacy and cultural awareness skills to effectively coordinate patient centric care.
• Basic understanding of managed care principles.
• Understanding Medicare and MediCal / Medicaid benefits.
• Basic Microsoft Office skills.
• Demonstrated ability to work with multiple computer platforms simultaneously.
• Proven excellent verbal and written communication skills with ability to read, write, speak, and understand English clearly.
• Bilingual in English and needed language (based upon demographic needs of panel).

Responsibilities

• Welcome and orient prospective patients to health enhancement and coordinating delivery of quality services.
• Act as primary point of contact to facilitate effective patient communication.
• Coordinate ongoing care with appropriate resource allocations, referrals, and notifications.
• Notify the Health Enhancement/Education team of patient requests and referrals.
• Facilitate enrollment to health enhancement programs and community-based support.
• Provide scripted health education and monitoring.
• Expedite referrals and scheduling and coordinate with team for follow ups.
• Meet or exceed productivity targets.
