Job summary

Guideway Care is seeking a Care Guide to support patients and their caregivers by connecting them with resources and resolving issues that impact their health. This 100% remote position involves outreach to patients after clinical events and collaboration with a clinical team to enhance patient experiences.

The ideal candidate will possess strong communication skills and technical ability, with a preference for a Bachelor’s degree. The pay range is $16 to $18 per hour, with benefits including medical and retirement plans.

Qualifications

• Proficiency with a personal computer and technical skills.
• Strong communication, interpersonal, and organizational skills are necessary.
• Experience with Microsoft Excel preferred.

Responsibilities

• Support patients and caregivers in resolving issues.
• Develop relationships with patients through telephonic outreach.
• Collaborate with the clinical team to improve patient experience.
• Research new community resources to assist patients.
• Empower patients to communicate their preferences.
