Job summary

Guidehealth is seeking a Care Manager to guide patients and families through complex health journeys. This remote role blends clinical expertise with holistic care to address medical, behavioral, and social needs.

You will build relationships, create personalized care plans, coordinate care across providers, and educate members to navigate the system confidently, with an emphasis on whole-person health and compassionate coordination.

Qualifications

• Active RN license in good standing.
• 5+ years in care management or chronic condition care.
• Must be local to St. Tamany Parish, LA.
• Remote work capability and independent work.
• Preferred: CCM certification.

Responsibilities

• Build relationships with patients and families and understand goals.
• Create personalized care plans addressing social determinants.
• Coordinate care across providers and settings.
• Educate and empower members to navigate care decisions.
• Collaborate with an interdisciplinary team for whole-person health.
• Use data to identify opportunities and measure impact with patient at center.
• Maintain regulatory compliance with compassion and connection.
