Job summary

Guidehealth is seeking a Community Resource Guide to bridge patients, physicians, and community resources, coordinating care across medical, behavioral health, substance use, and social services. This role supports whole-person health and improves health outcomes through navigation, assessment, and referrals.

You will engage patients remotely and in person, identify needs, follow care plans, and document interactions in EHR systems.

Qualifications

• At least 3 years of related experience in patient navigation, community resource navigation, or similar roles.
• Experience using EHR systems or similar documentation platforms.
• Ability to work effectively in a remote environment.

Responsibilities

• Build trusting relationships with patients, families, and care teams.
• Assist patients with referrals, transportation, scheduling, and community resources.
• Document interactions and outcomes in EHR systems.
• Support coordination to close care gaps and improve quality measures.
