Job summary

Guidehealth is a fully remote organization seeking a Care Manager to guide patients and families through complex health journeys from a Louisiana base. You will combine clinical expertise with holistic care, coordinating medical, behavioral, and social needs while educating members to navigate care decisions confidently.

Requirements include a minimum of 5 years as a Registered Nurse and an active Compact RN license. A CCM certification is a plus, and you must be local to St.

Qualifications

• Minimum of 5 years as a Registered Nurse.
• Active Compact RN License.
• Strong background in care management and chronic condition management.
• Motivational interviewing, patient education, and patient advocacy.
• Ability to work independently in a remote environment while staying connected to the team.
• Comfort with technology and multiple EMR systems.
• Must be local to St. Tamany Parish, LA.

Responsibilities

• Build meaningful relationships with patients and families, understanding goals, barriers, and motivators.
• Create personalized care plans addressing social determinants and emotional well-being.
• Coordinate care across providers and settings for safety, quality, and continuity.
• Educate and empower members to navigate the healthcare system confidently.
• Collaborate with an interdisciplinary team and advocate for whole-person health.
• Use data to identify opportunities and measure impact while centering the patient.
• Maintain compliance with regulatory standards with compassion.
