Job summary

SCAN is a nonprofit health organization dedicated to improving care for older adults across multiple states. This remote role operates within Pacific Time business hours, supervising care management programs to deliver high-quality, evidence-based care.

You will lead care management supervisors and pods, drive operational excellence, and ensure compliance with CMS, DMHC, and DHCS standards while fostering collaboration with providers and internal teams.

Qualifications

• Bachelor’s Degree or equivalent experience in healthcare operations or management.
• Graduate or advanced degree is preferred.
• Experience leading care management or population health programs.

Responsibilities

• Oversee care management teams and pods to drive operational excellence.
• Ensure compliance with CMS, DMHC, DHCS, and NCQA standards.
• Develop dashboards and reports to monitor performance and outcomes.
• Collaborate with internal departments to align clinical and operational goals.
• Mentor supervisors and build a culture of accountability and growth.
