Job summary

SCAN Health Plan seeks a Manager, Care Coordination to lead teams delivering education and navigation support for members, ensuring adherence to regulatory requirements and best practices. You will drive care coordination processes, oversee staffing and performance, and partner with stakeholders to meet organizational goals.

This role emphasizes regulatory compliance, quality standards, and continuous improvement while supporting a mostly remote work mode and a comprehensive benefits package.

Qualifications

• Bachelor's Degree or equivalent experience.
• 5+ years of experience in healthcare, medical management, change management, and/or leadership.
• Working knowledge of NCQA standards, SNP requirements, and CMS/DHS regulations.
• Knowledge of project and program management principles.
• Ability to lead and influence those without direct reporting relationships.
• Strong written and verbal communication skills.
• Advanced proficiency with Microsoft Word, Excel, PowerPoint, Outlook, Teams, and Visio.

Responsibilities

• Manage teams through oversight of day-to-day operations and productivity to meet department goals.
• Oversee workflow, priorities, and projects to ensure timely completion.
• Recruit, orient, train, and develop staff; maintain a safe, compliant environment.
• Support regulatory compliance with CMS/DMHC/DHCS/NCQA standards.
