The role emphasizes high-level patient service, coordination of care, and ongoing support by phone and email from a remote/home office. You will manage a patient census, stay informed on disease states and therapies, and collaborate with multiple teams to ensure timely medication orders and clinical needs are met.

Qualifications

• Must have customer-facing experience in a healthcare or similar setting.
• Experience coordinating patient care, scheduling, and communications.

Responsibilities

• Serve as liaison between patients, nursing partners and departments.
• Provide high-level patient service coordination and support to the department.
• Maintain patient census and manage care coordination for 120–130 patients.
• Understand disease states and medications relevant to home administration.
• Respond to recalls and coordinate with appropriate teams.
