Job summary

Covera Health is seeking a Patient Advocacy Specialist on the Medmo team to help Medicaid members access resources and support to improve their health and well-being.

You will educate members about available resources, connect them with community-based organizations, make referrals, follow up to ensure successful connections, and document outreach and outcomes with care and precision in a remote-first environment across the United States.

Qualifications

• Spanish fluency (written and spoken) is required.
• Experience in healthcare, patient services, or community health is preferred but not required.
• Experience in high-volume customer-facing roles (call center, patient services) is required.
• A proactive, self-motivated approach with resilience to navigate fast-paced environments.
• Strong judgment, empathy, and ability to balance multiple needs.

Responsibilities

• Engage members to understand health-related social needs and barriers.
• Educate and connect members with community resources and CBOs.
• Make referrals based on individual needs, eligibility, and preferences.
• Follow up to ensure successful connections and positive experiences.
• Document outreach, needs, referrals, and outcomes accurately and securely.
• Meet team performance goals while maintaining quality and empathy.
