Job summary

Graham Healthcare Group is hiring a Palliative Patient Specialist to support patients remotely across the United States. The role partners with field staff and leaders to provide compassionate, coordinated care and superior service.

Compensation is $19.00–$20.00 per hour, with standard hours Monday–Friday, 8AM–5PM. Benefits include health, vision, dental, 401K with company match, and 15 days PTO. Fully remote position with emphasis on HIPAA compliance and patient advocacy.

Qualifications

• HS diploma or GED.
• 2 years Hospice/Home Health experience preferred.
• Medical terminology knowledge preferred.

Responsibilities

• Develop and nurture referrals with accurate documentation.
• Manage channels like Teams, email, and Salesforce.
• Serve as primary contact for patients and families.
• Maintain Salesforce list including patient charts and assignments.
• Demonstrate excellent customer service internally and externally.
• Explain HIPAA and confidentiality requirements clearly.
• Monitor census for patients not current with another service line.
• Perform phone-based screenings to assess eligibility for levels of care.
• Conduct educational sessions by phone to maintain relationships with patients/MPOA.
• Collaborate with Palliative, Home Health, Journey and Hospice teams to expedite access to care.
• Act as liaison between company and patients/caregivers.
• Respond to web inquiries for service information.
• Adhere to company values and core competencies.
• Perform other duties as assigned and ensure regulatory compliance.
