Graham Healthcare Group is seeking a Palliative Patient Specialist to support our field teams and improve patient care. This remote, daytime role focuses on referrals, documentation, and patient advocacy across multiple service lines.

Responsibilities include outreach via Teams, phone, and Salesforce, maintaining patient charts, and ensuring HIPAA and confidentiality standards. Competitive benefits and 401k with company match are offered.

Qualifikationen

• High School diploma or GED required.
• 2 years Hospice and/or Home Health experience preferred.
• Medical Terminology knowledge preferred.
• Active listening and customer service skills required.

Aufgaben

• Develop and nurture referrals with accurate documentation.
• Manage communications via Teams, emails, and Salesforce.
• Serve as primary contact for patients and families.
• Maintain Salesforce lists including non-admitting/discharging charts and patient assignments.
• Demonstrate excellent customer service, both internally and externally.
• Understand HIPAA and client confidentiality requirements and stay compliant.
• Manage census of patients not current with another service line and related care.
• Perform screenings/needs assessments over the phone to determine eligibility for levels of care.
• Deliver educational sessions by phone to maintain relationships with patients/MPOA.
• Collaborate with Palliative, Home Health, Journey and Hospice teams to expeditate access to care.
• Serve as liaison between the company and patients/caregivers.
• Respond to web inquiries for service information as needed.
• Adhere to company Core Values and Core Competencies.
• Perform other duties as assigned and ensure compliance with regulations.
• Ensure compliance with local, state, and federal regulations.
