Job summary

Graham Healthcare Group is seeking a Hospice Journey Counselor to educate and support patients, families, and caregivers about end-of-life planning and hospice care. This remote, full-time role focuses on guiding eligible patients through transitions to hospice services and coordinating with clinicians to ensure compassionate, comprehensive care.

The position offers a fixed base compensation with a cap, bonuses, mileage reimbursement, and solid benefits, including medical, retirement, and PTO.

Qualifications

• High school diploma or GED required.
• Home healthcare experience preferred.
• Understanding of hospice philosophy preferred.
• Valid driver’s license required; you may drive ~20% of the time.

Responsibilities

• Manage Journey patient census and referrals from Journey candidate list and other sources.
• Review patient charts to determine hospice eligibility and prepare for conversations with patients/families.
• Make initial calls to new referrals to schedule home visits and follow up on patient status.
• Conduct home visits to discuss hospice benefits and build rapport with patients and families.
• Maintain Journey charts and notes in the system.
• Collaborate with Clinicians to address patient needs and ensure coordination.
• Ensure Journey charts exist for each patient.
• Meet or exceed weekly patient visits and calls and monthly transitions to hospice care targets.
• Assist clinicians in explaining hospice philosophy and benefits to patients and families.
• Support efficient transitions to Hospice when needed.
