Job summary

Graham Healthcare Group is hiring a Palliative Patient Specialist to partner with field staff and leaders, supporting patients and families across the continuum. You will manage referrals, documentation, and communications while ensuring HIPAA and corporate compliance.

The role is fully remote nationwide in the USA, with standard 40-hour weeks and benefits including medical, retirement, and PTO. A strong focus on customer service and care coordination is essential.

Qualifications

• High School diploma or GED is required.
• 2 years Hospice and/or Home Health experience preferred.
• Medical Terminology knowledge preferred.
• Active listening skills required.
• Customer service skills essential.
• Patience, dependability, good judgement and ability to manage concurrent demands.
• Proficient in Microsoft Office suite including Teams.
• Detail-oriented with strong time management.

Responsibilities

• Develop and nurture referrals while ensuring accurate documentation.
• Manage communications channels for the team (Teams, emails, Salesforce).
• Serve as primary contact and advocate for patients and families.
• Maintain updated Salesforce lists with patient assignments.
• Provide excellent internal and external customer service.
• Explain HIPAA and compliance requirements clearly.
• Manage census for patients not current with another service line.
• Perform screenings and needs assessments over the phone.
• Deliver educational sessions by phone to maintain relationships.
• Collaborate with Palliative, Home Health, Journey and Hospice teams to expedite access to care.
• Serve as liaison between company and patients/caregivers.
• Respond to web inquiries for service information.
• Adhere to Core Values and Core Competencies.
• Perform other duties as assigned; ensure regulatory compliance.
