Job summary

Graham Healthcare Group is seeking a Nurse Practitioner specialized in palliative care to provide in-home patient-centered services in the Plymouth area. This role emphasizes autonomous practice with strong collaboration with interdisciplinary teams and ongoing professional growth.

The position requires a BSN and MSN with at least five years of clinical experience, active RN/NP licensure in the state, and preferred experience in home health or hospice/palliative care.

Qualifications

BSN and MSN degrees required.
Minimum of five years of clinical experience.

Experience in a community-based Home Health or Hospice/Palliative Care setting preferred.

Responsibilities

Deliver patient-centered palliative care in home settings with families and interdisciplinary teams.
Perform comprehensive assessments and manage symptoms of serious illness.
Develop individualized treatment plans addressing physical, emotional, and psychosocial needs.
Facilitate goals-of-care discussions and educate patients and caregivers.
Travel to patient homes within the assigned territory and coordinate care with EMR documentation.

What You’ll Do

Deliver compassionate, patient-centered palliative care, partnering with patients, families, and interdisciplinary teams to improve quality of life and support goal-concordant care
Perform comprehensive assessments, manage symptoms associated with serious illness, and develop individualized treatment plans that address physical, emotional, and psychosocial needs
Facilitate goals-of-care discussions, educate patients and caregivers on treatment options, and apply advanced clinical judgment to coordinate care and maintain thorough, compliant EMR documentation
Work independently, but never alone
Travel to patient homes within assigned territory

Nurse Practitioner Qualifications

Bachelor of Science (BSN) degree from an accredited School of Nursing
Master of Science (MSN) degree from an accredited School of Nursing
A minimum of five (5) years of clinical experience
Hold, in good standing, a license as a Registered Nurse and Nurse Practitioner issued by the state in which you work
Experience in a community based Home Health or Hospice/Palliative Care practice is strongly preferred
