Qualifications

• Minimum 2 years in palliative, hospice, or home care.
• Bachelor’s degree in a health-related field preferred.
• Experience with medical records and data analysis in EHR.

Responsibilities

• Coordinate, schedule, and assign palliative patients to providers.
• Follow up with patients and families to collect data on care plan response.
• Act as contact point for patients, care team, families, payers, and resources.
• Manage referrals to agencies to support care plan goals.
• Assist patients with barriers related to healthcare access, interpreters, transportation, or prescriptions.
