Job summary

Graham Healthcare Group is seeking a Hospice Journey Counselor to educate patients and families about end-of-life planning and hospice care. The role includes managing patient referrals, conducting home visits, and coordinating with clinicians to ensure smooth transitions to hospice services.

Responsibilities include reviewing eligibility, making initial contact with referrals, and maintaining comprehensive patient charts while meeting performance expectations for visits and calls.

Qualifications

• High school diploma or GED required.
• Valid driver’s license and ability to drive ~20% of time.
• Prior home healthcare experience preferred.
• Understanding of hospice philosophy preferred.

Responsibilities

• Manage Journey patient census and referrals for the assigned area.
• Review patient charts to determine eligibility for hospice benefits.
• Make initial calls to referrals to schedule home visits and follow-up contacts.
• Conduct patient visits to discuss hospice benefits with patient/family.
• Maintain patient charts with notes from visits and calls.
• Communicate with clinicians to coordinate patient needs.
• Ensure Journey charts are created and maintained.
• Strive to meet weekly patient visits and calls, monthly hospice transitions.
• Assist clinicians in explaining hospice philosophy and benefits.
• Support transitions to Hospice as needed.
