Job summary

Graham Healthcare Group is hiring a Journey Administrative Specialist to support Hospice's face-to-face needs and to coordinate referrals across branches, ensuring timely and accurate documentation.

The role requires excellent customer service, HIPAA compliance awareness, and proficiency with Teams, emails, and Salesforce. Remote work is possible, but candidates must be based near Troy, MI, Mechanicsburg, PA, or Warrendale, PA.

Qualifications

• High School diploma or GED completed.
• Experience in Journey counseling or hospice settings preferred.
• Medical terminology knowledge is required.
• Excellent listening and customer service skills.
• Strong time management and attention to detail.
• Proficiency with Microsoft Office and Teams.
• Ability to handle HIPAA and client confidentiality.

Responsibilities

• Develop and process journey referrals with accurate documentation.
• Manage chart closure processes and provide requested documentation.
• Communicate via Teams, email, SF, and referral portals.
• Assist Journey team members with patient contact across branches.
• Deliver excellent internal and external customer service.
• Ensure HIPAA compliance and confidentiality across duties.
• Collaborate with Palliative, Home Health, and Hospice teams as needed.
