Job summary

Valeris seeks a dedicated Care Coordinator to manage patient cases from start to finish, handling benefit investigations, authorizations, and financial qualification determinations. You will assist patients and healthcare providers via calls and email using our Lynk platform, ensuring timely resolutions and adherence to program rules.

The role emphasizes ownership, attention to detail, and patient-focused service in a fast-paced environment.

Qualifications

• Bachelor’s degree strongly preferred or equivalent experience.
• Customer service and healthcare industry experience preferred but not required.
• Ability to work an 8.5-hour shift between 8 am to 9 pm, Monday to Friday.
• Bi-lingual skills in Spanish and English are a plus.

Responsibilities

• Ownership of patient cases from initiation to closure, including benefit investigations and prior authorizations.
• Assist patients and callers via email and phone using the Lynk system.
• Determine financial qualification for assistance programs and resolves inquiries.
• Coordinate nurse teach with field-based nurse educators and support adherence services.
• Document activities in the Lynk system and follow SOPs.
