Job summary

Wentworth Hospice, Amedisys a part of LHC Group, within the Optum family, seeks a Hospice On-call Registered Nurse for Mon-Thu 5pm–8am to deliver after-hours clinical support, triage patient needs, and coordinate care across home health and hospice teams.

This role blends remote and field-based nursing, enabling education for patients and caregivers while ensuring consistent documentation and collaboration with physicians to achieve timely, quality outcomes.

Qualifications

• Current RN license specific to the state of assignment.
• Current CPR certification.
• 1+ years of nursing experience.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Provide on-call nursing support, triaging patient needs and determining appropriate interventions.
• Assess patient conditions via phone and/or in-person visits and upscale care as needed.
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery.
• Deliver patient and caregiver education related to symptom management and care plans.
• Maintain accurate and timely documentation of all patient interactions and clinical decisions.
