Job summary

ComplexCare Solutions seeks a Nurse Practitioner to perform comprehensive home and telehealth assessments regionally, delivering patient-centered care and detailed documentation. You will spend time per member to understand personal needs and create evidenced-based treatment plans to keep individuals in their homes and out of the hospital.

The role requires an active NP license with family/Adult/gerontology or emergency medicine certification, and proficiency with EHRs, CMS guidelines, and CPT-4

Qualifications

• Active un-encumbered nursing license.
• ANCC or AANP board certification as a Nurse Practitioner in Family, Adult, Gerontology or Emergency Medicine.
• Knowledge of CMS Regulations and NCQA HEDIS Guidelines.
• Familiarity with ICD-10 and CPT-4 coding practices.
• Proficient with electronic health records (EHRs) and secure messaging.

Responsibilities

• Conduct comprehensive home and telehealth assessments with patients.
• Document findings and recommendations in the medical record.
• Coordinate with physicians and health plans to manage chronic conditions.
• Identify care gaps and suggest evidence-based interventions.
• Maintain HIPAA compliance and confidentiality at all times.
