ComplexCare Solutions is seeking a Nurse Practitioner to provide per diem home and telehealth assessments across Fairfield County, CT. The role involves comprehensive patient evaluations, documentation, and collaboration with PCPs and health plans.

The ideal candidate holds an active NP license with ANCC/AANP certification in Family, Adult, Gerontology or Emergency Medicine, and CPR. Flexible hours and a path to ongoing engagement are offered.

Qualifications

• Active un-encumbered NP license.
• ANCC or AANP board certification in Family, Adult, Gerontology or Emergency Medicine.
• Maintains current CPR certification.
• Familiarity with CMS Regulations and NCQA HEDIS Guidelines.
• Ability to perform home and telehealth assessments with thorough documentation.

Responsabilités

• Conduct comprehensive home and telehealth NP assessments.
• Document findings and formulate evidence-based treatment plans.
• Address gaps in care and coordinate with PCP and health plans.
• Maintain HIPAA compliance and protect patient confidentiality.
• Provide patient education to improve chronic condition management.
• Maintain at least 25 available slots per month (~37 hours).
• Communicate with physicians and health plans regarding follow-up.
