Job summary

Mercy is seeking an experienced Registered Nurse to join our team as a Nurse Navigator. The role guides patients through their healthcare journey, coordinating care from pre-treatment to follow-up, and serves as a primary resource for patients and families.

The position is part-time and hybrid, with satellite or remote coordination as needed. You will work with physicians and interdisciplinary teams to ensure safe, high-quality care while supporting discharge planning and utilization management.

Qualifications

• RN license in the state of practice required.
• Minimum of 2-3 years of nursing experience.
• BLS certification required; ACLS certification preferred.

Responsibilities

• Coordinate care for assigned inpatient and outpatient populations.
• Guide patients through their treatment journey, serving as a primary resource and advocate.
• Provide patient and family education, including pre-operative preparation and follow-up care.
• Collaborate with physicians, nurses, and multidisciplinary teams to ensure high-quality, patient-centered care.
• Support discharge planning and coordinate post-acute services.
• Monitor patient progress and help improve outcomes, patient satisfaction, and continuity of care.
• Assist with length of stay (LOS) management and utilization review activities.
• Participate in quality improvement initiatives and implementation of best practices.
• Serve as a clinical resource and mentor to members of the care team.
• Maintain accurate documentation within the electronic medical record (EPIC).
