Job summary

Mercy seeks a Nurse Navigator (Part-Time, Hybrid) to guide patients through their healthcare journey, coordinate care from pre-treatment to follow-up, and collaborate with a multidisciplinary team to ensure seamless, patient-centered care. The role emphasizes discharge planning, education, and utilization management in a remote-friendly hybrid setting.

The ideal candidate will be an experienced RN with 2–3 years of nursing experience, a current RN license, BLS, and preferred BSN. ACLS is a plus.

Qualifications

• Graduate of an accredited School of Nursing.
• Current RN license in the state of practice.
• Minimum of 2-3 years of nursing experience.
• BLS certification.

Responsibilities

• Coordinate care for inpatient and outpatient populations.
• Guide patients through their treatment journey as primary resource and advocate.
• Provide patient and family education including pre-operative prep and follow-up care.
• Collaborate with physicians, nurses, and multidisciplinary teams to ensure high-quality care.
• Support discharge planning and coordinate post-acute services.
• Monitor patient progress and strive to improve outcomes and satisfaction.
• Assist with LOS management and utilization review activities.
• Participate in quality improvement initiatives and best practices implementation.
• Serve as a clinical resource and mentor to care team members.
• Maintain documentation in EPIC.
