Job summary

Mercy is seeking an experienced RN to join as Nurse Navigator on a part-time, hybrid schedule. You will guide patients through their care journey, coordinate inpatient and outpatient services, educate families, and collaborate with physicians and care teams to ensure high-quality, patient-centered outcomes.

Qualifications

• Graduate of an accredited School of Nursing with an active RN license.
• BLS certification required.
• Preferred BSN degree and ACLS certification.

Responsibilities

• Coordinate care for assigned inpatient and outpatient populations.
• Guide patients through their treatment journey, serving as a primary resource and advocate.
• Provide patient and family education, including pre-operative preparation and follow-up care.
• Collaborate with physicians, nurses, and multidisciplinary teams to ensure high-quality, patient-centered care.
• Support discharge planning and coordinate post-acute services.
• Monitor patient progress and help improve outcomes, patient satisfaction, and continuity of care.
• Assist with length of stay (LOS) management and utilization review activities.
• Participate in quality improvement initiatives and implementation of best practices.
• Serve as a clinical resource and mentor to members of the care team.
• Maintain accurate documentation within the electronic medical record (EPIC).
