Qualifications

• Graduate of an accredited School of Nursing.
• Current RN license in the state of practice.
• Minimum of 2-3 years of nursing experience.
• BLS certification.

Responsibilities

• Coordinate care for inpatient and outpatient populations.
• Guide patients through treatment journeys and act as advocate.
• Provide patient and family education including pre-operative prep and follow-up.
• Collaborate with physicians and nurses to ensure high-quality care.
• Support discharge planning and post-acute services.
• Monitor progress to improve outcomes and satisfaction.
• Assist with LOS management and utilization review.
• Participate in quality improvement and best-practice adoption.
• Serve as a clinical resource and mentor to care team.
• Maintain documentation in EPIC EMR.
