Job summary

Mercy is seeking a Part-Time Nurse Navigator to guide patients through their healthcare journey, coordinating care from pre-treatment to follow-up. This role emphasizes collaboration with physicians, nurses, and interdisciplinary teams to deliver high-quality, patient-centered care.

The position offers hybrid work and opportunities to impact outcomes and patient experience while supporting discharge planning and utilization management within Mercy's care model.

Qualifications

• Graduate of an accredited School of Nursing.
• Current RN license in the state of practice.
• Minimum of 2-3 years of nursing experience.
• BLS certification.

Responsibilities

• Coordinate care for assigned inpatient and outpatient populations.
• Guide patients through their treatment journey as a primary resource and advocate.
• Provide patient and family education, including pre-operative preparation and follow-up care.
• Collaborate with physicians, nurses, and multidisciplinary teams to ensure high-quality care.
• Support discharge planning and coordinate post-acute services.
• Monitor patient progress and help improve outcomes, patient satisfaction, and continuity of care.
• Assist with LOS management and utilization review activities.
• Participate in quality improvement initiatives and implementation of best practices.
• Serve as a clinical resource and mentor to care team members.
• Maintain accurate documentation within EPIC.
