Mercy seeks an experienced Nurse Navigator to coordinate care across inpatient and outpatient settings, educate patients and families, and partner with physicians and care teams. The role emphasizes discharge planning, utilization management, and quality improvement while guiding patients through pre-treatment and post-treatment care in a hybrid, part-time capacity.

Competenze

• RN license in state of practice
• Graduate of an accredited School of Nursing
• Minimum of 2-3 years of nursing experience
• BLS certification
• BSN preferred
• ACLS certification (preferred)

Mansioni

• Coordinate care for inpatient and outpatient populations
• Guide patients through treatment journey as primary resource
• Provide patient and family education including pre-operative prep and follow-up
• Collaborate with physicians, nurses and multidisciplinary teams
• Support discharge planning and post-acute services
• Monitor patient progress and improve outcomes and satisfaction
• Participate in quality improvement initiatives and best practices
• Maintain accurate EPIC documentation
