Mercy is seeking a part-time Nurse Navigator to guide patients through their healthcare journey, coordinating care from pre-treatment to follow-up. This role partners with patients, families, physicians, and care teams to assess needs, develop care plans, and connect patients with resources, education, discharge planning, and quality initiatives.

The ideal candidate is a registered nurse with 2–3 years of nursing experience, BLS, and ideally BSN/ACLS.

Competenze

• Graduate of an accredited School of Nursing.
• Current RN license in the state of practice.
• Minimum of 2-3 years of nursing experience.
• BLS certification.
• ACLS certification (preferred).
• BSN (preferred).

Mansioni

• Coordinate care for assigned inpatient and outpatient populations.
• Guide patients through their treatment journey, serving as a primary resource and advocate.
• Provide patient and family education, including pre-operative preparation and follow-up care.
• Collaborate with physicians, nurses, and multidisciplinary teams to ensure high-quality, patient-centered care.
• Support discharge planning and coordinate post-acute services.
• Monitor patient progress and help improve outcomes, patient satisfaction, and continuity of care.
• Assist with length of stay (LOS) management and utilization review activities.
• Participate in quality improvement initiatives and implementation of best practices.
• Serve as a clinical resource and mentor to members of the care team.
• Maintain accurate documentation within the electronic medical record (EPIC).
