Mercy seeks an experienced Registered Nurse to join as Nurse Navigator. This part-time, hybrid role guides patients through the healthcare journey, coordinates care, and improves outcomes from pre-treatment to follow-up.

You will work with physicians, bedside nurses, and interdisciplinary teams to ensure seamless care, discharge planning, and utilization management. The ideal candidate has an accredited nursing degree, current RN license, 2–3 years of nursing experience, and BLS.

Formación

• Graduate of an accredited School of Nursing.
• Current RN license in the state of practice.
• Minimum of 2-3 years of nursing experience.
• BLS certification.

Responsabilidades

• Coordinate care for assigned inpatient and outpatient populations.
• Guide patients through their treatment journey as primary resource and advocate.
• Provide patient and family education, including pre-operative prep and follow-up care.
• Collaborate with physicians, nurses, and multidisciplinary teams to ensure high-quality, patient-centered care.
• Support discharge planning and coordinate post-acute services.
• Monitor patient progress and help improve outcomes and satisfaction.
• Assist with LOS management and utilization review activities.
• Participate in quality improvement initiatives and best practices.
• Serve as a clinical resource and mentor to care team members.
• Maintain accurate documentation in EPIC.
