Job summary

Mercy is seeking a Part-Time Nurse Navigator hybrid role to guide patients through the healthcare journey, coordinating care from pre-treatment through follow-up. You will work closely with families, physicians, bedside nurses, and interdisciplinary teams to assess needs, plan care, and connect patients with needed resources.

Qualifications

• RN license required in state of practice.
• Minimum 2-3 years of nursing experience.
• BLS certification required.

Responsibilities

• Coordinate care for assigned inpatient and outpatient populations.
• Guide patients through their treatment journey as primary resource and advocate.
• Provide patient and family education, including pre-operative prep and follow-up care.
• Collaborate with physicians, nurses, and multidisciplinary teams to ensure high-quality, patient-centered care.
• Support discharge planning and coordinate post-acute services.
• Monitor patient progress and help improve outcomes and satisfaction.
• Assist with LOS management and utilization review activities.
• Participate in quality improvement initiatives and best practices implementation.
• Serve as a clinical resource and mentor to care team members.
• Maintain accurate documentation in EPIC.

Education

Graduate of an accredited School of Nursing
