Job summary

Mercy is seeking a Nurse Manager to lead the Medical Management & Transition Team, overseeing virtual-based transitional care programs and ensuring safe, high-quality, and coordinated patient care across ED-to-Home pathways.

The role emphasizes operational excellence, performance improvement, and population health strategies, with a focus on team collaboration and regulatory compliance to advance Mercy’s mission.

Qualifications

• Graduate of an approved school of nursing.
• Current licensure by the State Board of Nursing in the applicable state of practice.
• Four (4) years of experience in clinical specialty or related area; ability to apply management, supervision, teaching, performance improvement, and budget concepts.
• CPR and NRP certifications.
• Medication and math competency test required before administering medications.

Responsibilities

• Provide operational, clinical, and strategic leadership for virtual-based transitional care programs.
• Ensure safe, effective, patient-centered care delivery and timely follow-up across the care continuum.
• Foster interdisciplinary collaboration and monitor quality outcomes and regulatory compliance.
