Job summary

Mercy is seeking a Nurse Manager to lead its virtual-based transitional care medical management and Mercy Lifestyle and Wellness programs. You will drive daily operations, ensure quality outcomes, supervise staff, and maintain regulatory compliance across ED-to-Home and chronic condition follow-up initiatives.

Expect to guide care coordination, promote patient-centered care, and collaborate with interdisciplinary teams while advancing Mercy’s mission through innovative virtual care strategies.

Qualifications

• Graduate of an approved school of nursing.

Responsibilities

• Provide operational, clinical, and strategic leadership for virtual-based transitional care programs.
• Ensure safe, effective, and patient-centered care delivery; foster interdisciplinary collaboration.
• Oversee daily operations, quality outcomes, staff performance and regulatory compliance of services.
• Advance Mercy’s mission through population health strategies and high-quality virtual care management.
