Mercy is seeking a Nurse Manager for the Medical Management & Transition Team to provide operational, clinical, and strategic leadership for Mercy’s virtual-based transitional care programs. This role oversees the daily operations, quality outcomes, staff performance, and regulatory compliance to support patients moving through the care continuum.

As a BSN with active RN licensure and several years of supervisory experience, you will drive safe, patient-centered care, foster collaboration

Qualifications

• Graduate of an approved school of nursing with licensure in the state of practice.
• Current licensure by the State Board of Nursing.
• Four years of clinical experience with management, supervision, or performance improvement.
• CPR and NRP certifications required.
• Medication and math competency test before medication administration duties.
• BS/MS preferred; supervisory/management experience preferred.

Responsabilités

• Provide operational, clinical, and strategic leadership for virtual transitional care and wellness programs.
• Oversee daily operations, quality outcomes, staff performance, and regulatory compliance.
• Foster interdisciplinary collaboration and ensure timely follow-up and care coordination.
• Advise on population health strategies and scalable care models.
