Responsibilities

• Deliver remote patient education via telephonic, video, email, SMS and secure messaging.
• Provide in-person education during territory coverage, including home visits.
• Coordinate connections to program-approved patient resources and facilitate dialogue with healthcare providers.
• Maintain accurate documentation and adhere to approved protocols and KPIs.
• Collaborate with District Managers and Field Leadership; participate in training and onboarding new team members.
