Job summary

ComplexCare Solutions is recruiting a Nurse Practitioner for per diem assignments across home visits. You will conduct comprehensive assessments in patients' homes, document findings, and develop individualized care plans to keep members healthy and out of the hospital.

The position requires active NP license, ANCC/AANP certification, CPR, and familiarity with CMS/NCQA guidelines. Telehealth options may be available; in-home visits are compensated per visit with a monthly earnings potential.

Qualifications

• Active unencumbered NP license.
• ANCC or AANP board certification in Family/Adult/Gerontology/Emergency Medicine.
• Current CPR certification.
• Knowledge of CMS regulations and NCQA guidelines.

Responsibilities

• Perform member assessments and document findings in the EHR.
• Develop evidence-based treatment plans and follow up.
• Address gaps in care and coordinate with PCPs and health plans.
• Maintain HIPAA compliance and patient confidentiality.
• Provide telehealth where applicable and in-home visits.
