Job summary

Chapters Health System, Inc. seeks a Resource Nights & Weekend Nurse Practitioner to provide fully remote virtual serious illness care management across our continuum of care. The role covers adult and pediatric hospice, palliative care, and related programs.

The NP must have 3+ years in hospice/palliative or acute inpatient care, hold an NP degree, and be licensed with prescriptive authority in applicable states. Strong EMR skills and collaboration with the IDT are required.

Qualifications

• Must hold a Masters or Doctors degree in Nursing from an accredited Family or Adult Nurse Practitioner Program.
• Minimum of 3 years of recent clinical experience, including hospice/palliative or acute inpatient care.
• Must hold national certification, licensure, and prescriptive authority for advanced nursing practice; licenses must be obtained in all required jurisdictions.

Responsibilities

• Provide fully remote virtual serious illness care management to patients and families.
• Consult with medical staff to ensure optimal patient care.
• Perform virtual assessment of patient/family needs and adjust plan of care accordingly.
• Maintain EMR access and document interactions in a timely manner.
• Support discharge planning including prescribing meds and orders.
