Job summary

Chapters Health System, Inc. is seeking a Resource Nights & Weekend (ReNW) Nurse Practitioner to provide fully remote serious illness care management across our continuum of care, including home-based, facility-based, and inpatient hospice and palliative services. This role supports QAPI activities and requires cross-team collaboration.

The ideal candidate has a NP Masters/Doctorate, national certification, and an active compact RN license. Shifts may include nights, weekends, and holidays.

Qualifications

• Masters or Doctorate in Nursing from an accredited Family or Adult NP program.
• National certification, licensure, and prescriptive authority in home state and other states where care is provided.
• Basic Life Support certificate and compact RN license in home state by date of hire.

Responsibilities

• Provides fully remote virtual serious illness care management to patients and families across the continuum.
• Consults with medical staff to ensure optimal patient care.
• Performs virtual assessment of patient/family/caregiver needs.
• Identifies, manages, orders, and prescribes medications and equipment as appropriate.
• Documents interactions in the EMR in a timely manner.
