Job summary

ComplexCare Solutions is seeking a Nurse Practitioner per diem to conduct comprehensive in-home and remote assessments across Grafton County, NH. You will spend up to an hour with members to capture their personal needs and develop care plans to keep them out of the hospital.

The role emphasizes adherence to CMS/HEDIS guidelines, patient documentation, and effective communication with PCPs and health plans. Telehealth options and annual 401K enrollment are part of the package.

Qualifications

• Active un-encumbered nursing license.
• ANCC or AANP Nurse Practitioner certification.
• CPR certification is current.
• Compliance with CMS/DHHS regulations and vaccination/testing requirements.
• Experience with home health or remote patient assessments is a plus.

Responsibilities

• Conduct in-home or telehealth assessments to evaluate health status and care gaps.
• Document all findings accurately in the patient medical record.
• Communicate results and care recommendations to PCPs and health plans.
• Close identified gaps in care and recommend interventions.
• Perform Point of Care testing and document results.
• Ensure HIPAA compliance and maintain confidentiality at all times.
