Job summary

ComplexCare Solutions is seeking a Nurse Practitioner for per diem home and telehealth assessments across Barnstable County, MA. You will spend substantial time with each member to capture needs and develop care plans that keep patients at home and out of the hospital.

You will perform comprehensive reviews, document findings, and coordinate with physicians, health plans, and CMS/CMS guidelines while maintaining HIPAA compliance and high-quality patient service.

Qualifications

• ANCC or AANP board certification as a Nurse Practitioner in Family, Adult, Gerontology or Emergency Medicine.
• Maintains current CPR certification.
• Active un-encumbered nursing license and HIPAA compliance.
• Knowledge of CMS regulations, NCQA guidelines and ICD-10/CPT-4 coding.

Responsibilities

• Perform comprehensive member assessments in home or telehealth settings.
• Document assessment findings and develop evidence-based treatment plans.
• Coordinate with physicians, PCPs and health plans for timely follow-up.
• Ensure all medical conditions are documented per CMS and client requirements.
• Maintain HIPAA compliance and high-quality patient service.
