Placements24 is seeking a highly motivated Remote Case Manager for a leading non-profit focused on mental health advocacy. This fully remote role serves clients nationwide, requiring autonomous, compassionate virtual care, and strong organizational skills.

You will assess needs, develop care plans, coordinate referrals, and provide ongoing remote support while ensuring secure documentation and adherence to privacy standards.

Qualifications

• A relevant degree in Social Work, Psychology, Counselling, or related field.
• Regulatory registration with SACSSP or HPCSA.
• Minimum of 4 years in remote case management, preferably in mental health.

Responsibilities

• Conduct comprehensive needs assessments for clients seeking mental health support.
• Develop individualized care plans with clients, families, and healthcare providers.
• Facilitate referrals to therapy, psychiatric care, and support groups.
• Provide ongoing support, advocacy, and crisis intervention via phone, video calls, and secure messaging.
• Monitor progress and adjust care plans to achieve optimal outcomes.
• Maintain confidential client records per policy and data protection rules.
• Coordinate care with external agencies and service providers.
• Educate clients and families about mental health conditions and resources.
• Continuous improvement of case management processes and service delivery.
