Job summary

Placements24 is seeking an experienced Remote Social Support Case Manager to join our national team. The role is 100% remote, enabling you to support individuals and families from home while coordinating services and advocating for clients.

You will assess needs, develop care plans, connect clients with resources, and maintain confidential records using our digital system. A bachelor’s degree and 3+ years in case management are required.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology or a related field.
• Minimum of 3 years of experience in case management or social support services.
• Ability to work independently and manage caseloads remotely.
• Excellent verbal and written communication with active listening.
• Proficiency with digital case management software and standard office apps.

Responsibilities

• Assess client needs, including social, emotional, and practical support.
• Develop, implement, and monitor individualized case plans.
• Coordinate connections to community resources and healthcare providers.
• Provide ongoing guidance and advocacy throughout the service journey.
• Maintain confidential client records in the digital system.
• Communicate with clients via phone, video, and secure messaging.
• Collaborate with therapists, medical professionals, and agencies.
• Stay informed about social support programs, policies, and practices.
• Prepare progress reports and identify trends.
