Job summary

CNS Healthcare is seeking a Case Manager to join our community-based team in Michigan. You will coordinate client-centered services, conduct assessments, and develop care plans within a trauma-informed framework. The role supports mental health and substance use services for diverse populations.

Opportunity for some remote flexibility after onboarding. Compensation reflects experience and licensure level, with a comprehensive benefits package and PSLF eligibility options.

Qualifications

• QMHP credential required or equivalent licensure/credential.
• Bachelor’s degree in social work, sociology, counseling, psychology or related field.
• Preferred master’s degree in social work or related area.
• Proficient with Microsoft Office and electronic health records (EHR).

Responsibilities

• Coordinate client‑focused treatment and services in a trauma‑informed approach.
• Provide population and age‑appropriate case management and plan development.
• Conduct assessments, care coordination, and resource development for Individuals.
• Assist with medication support and coordination with healthcare providers.
• Complete documentation accurately and in line with CNS policy.
• Participate in weekly groups and ensure transportation assistance as needed.
