Job summary

Placements24 is seeking a dedicated Case Manager to join our expanding team. This remote role focuses on serving individuals in Welkom and the Free State region, coordinating services, and supporting clients to overcome challenges and achieve personal goals.

You will assess, plan, and connect clients with appropriate resources, ensuring timely and effective care in a client-centered approach. Independent work and technology-enabled delivery are essential.

Qualifications

Bachelor's degree in social/human services field.
Minimum 3 years of case management or similar client support experience.
Proficiency with case management software and virtual communication tools.
Strong understanding of community resources and social service systems.
Excellent organizational, time management, and problem-solving skills.
Ability to work independently, maintain confidentiality, and manage a remote caseload.

Responsibilities

Conduct thorough assessments of client needs, including psychosocial, medical, and financial factors.
Develop individualized case plans with clear goals, objectives, and timelines in collaboration with clients.
Coordinate and facilitate access to services such as healthcare, housing, employment, and counseling.
Monitor client progress regularly and adjust service plans as needed.
Maintain accurate electronic case records and documentation in line with organizational standards.
Act as a primary point of contact for clients, providing ongoing support, advocacy, and resource navigation.
