Job summary

Aptive is seeking a Medical Records Indexer to support the Mann-Grandstaff VA Medical Center remotely from Spokane, WA. The contractor will index approx. 24,800 records into Cerner/Oracle, ensuring accuracy and compliance with VA policies.

A minimum of 4 FTE is required to clear the backlog within the base period. Responsibilities include attaching images, linking FINs to veteran records, and QA checks to achieve 96% accuracy and productivity targets.

Qualifications

• Knowledge of clinical documentation and medical record workflows.
• Ability to locate FINs and link documents to correct veteran records.
• Ability to index documents with correct section, title, date of service, and visit location.
• Strong data entry skills with high accuracy and attention to detail.
• Familiarity with Cerner/Oracle EHR systems preferred.

Responsibilities

• Attach images to correct EHR elements and verify legibility.
• Index documents with accurate metadata and visit details.
• Perform 100% internal QA prior to submission.
• Maintain productivity targets (15 admin documents/hour; 10 NVCC/hour).
• Report errors to HIMS Supervisors promptly.
