Job summary

Aptive Resources seeks a Medical Records Indexer to support the Mann-Grandstaff VA Medical Center in a remote capacity. The role requires precise indexing of medical and administrative records into Cerner/Oracle, with a strong emphasis on accuracy and compliance with VA policies.

Responsibilities include attaching images, verifying uploads, linking FINs to veteran records, and maintaining QA standards. Prior experience with Cerner/Oracle or similar systems is preferred; training on VA security

Qualifications

Knowledge of medical clinics and clinical documentation required.
Ability to locate FINs and link documents to veteran records in Cerner/Oracle.
Ability to index documents in correct sections with correct metadata (title, date of service, visit location).
Images must be uploaded legibly and verifiably.
Strong data entry with high accuracy and attention to detail.
Experience with Cerner/Oracle or similar EHR systems preferred.

Responsibilities

Accurately attach images to the correct electronic health record elements.
Verify images are correctly uploaded and legible for viewing.
Locate FINs and link documents to the correct veteran record.
Index documents with proper section, note title, date of service, and visit location.
Perform 100% internal QA before submission.
Maintain productivity and accuracy targets per hour and month.
Report errors to supervisor promptly.
