Job summary

Responsibilities

• Conduct outbound calls with patients in English and Spanish to gather detailed medical histories.
• Listen actively and document information with structured templates and internal systems.
• Review medical records for completeness, clarity, and internal consistency.
• Organize and standardize medical information to meet quality and documentation standards.
• Ensure patient histories are thorough, consistent, and easy for physicians to interpret.
• Follow established workflows and documentation protocols.
• Partner with the operations team to continuously improve processes and quality.
