Job summary

One Senior Care Inc. is seeking a Medical Assistant – Clinical Documentation Support to organize records and prepare provider-facing documentation for visits. You will manage documentation templates, review queues, and ensure ready-to-review packets for practitioners in a remote role.

You will collaborate with CDI Nurses, Medical Records, and Clinical Operations, applying CC-focused coding and HIPAA compliance while maintaining neutral, factual documentation and supporting quality care.

Qualifications

• High School Diploma or GED required.
• Medical Assistant training or equivalent preferred.
• Previous medical coding with CC focus required.

Responsibilities

• Gather and organize existing medical records and approved clinical documentation.
• Prepare provider-facing documentation packets before visits.
• Review work queues and retrieve reports and supporting documentation.
• Organize hospital, ED, SNF, labs, imaging, medications, vital signs, and care plans.
• Ensure materials are complete, accurate, and timely.
• Track missing records and follow up through approved channels.
• Maintain accurate work queues and readiness logs.
• Escalate unclear or audit-sensitive information to clinical leadership.
• Use approved templates and neutral language in materials.
• Assist with follow-up tracking after provider visits.
• Participate in quality improvement and training activities.
