Job summary

One Senior Care is seeking a Clinical Documentation Integrity Nurse to support accurate, complete provider documentation for PACE Participants. You will review records, identify opportunities, and prepare neutral summaries to aid clinicians and ensure audit readiness.

The role requires CC-focused coding expertise, strong clinical judgment, and independent work in a developing CDI program. Remote schedule with standard business hours.

Qualifications

• Active, unrestricted RN license in the applicable state.
• Minimum of 3 to 5 years of clinical nursing experience.
• Experience in Clinical Documentation Integrity, provider documentation, coding, risk adjustment, or a similar role.
• Strong coding experience with CC focus.
• Ability to provide neutral, professional documentation support.

Responsibilities

• Review medical records and clinical intelligence reports to identify documentation opportunities for provider review.
• Review provider notes, hospital and skilled nursing records, meds, labs, imaging, assessments, care plans.
• Apply clinical documentation and coding knowledge with CC-focused coding.
• Prepare concise, neutral clinical summaries for provider review.
• Provide onboarding, training, and coaching to Medical Assistants.
